Social Prescribing Link Worker aka Life Changer Wanted – Thurrock CVS

Social prescribing is a means of enabling primary care services to refer patients with social, emotional or practical needs to a range of local, non-clinical services. This service is provided as part of a model of partnership between the voluntary and community sector (VCS) and GPs within Primary Care Networks.

This role is to develop a high quality customer care service to Multi-Disciplinary Teams and Patients within the Grays PCN (Thurrock) and responsible for liaison and the development of effective working relationship with health professionals and GP’s. You will also work alongside voluntary and community groups to provide patient support for GP practices.

 

Grays Primary Care Network are looking for a part time Social Prescribing Link Worker to work alongside their existing Full-Time Link Worker.

 

17.5 hours per week 

Needs to be flexible to meet the needs of the service, could involve some evening/weekend working

Salary £10,907 

 

Based within GP practices in the Grays area of Thurrock.

Car drivers with the use of a vehicle are preferable.  

This post will require an enhanced DBS.

 

Thurrock CVS is based at the Beehive, West Street, Grays, RM17 6XP with excellent links to the C2C trains and bus routes.
